For GP appointments

Either fill in a Form/Book online/Phone/In person

Fill in a form

  • Fill in the form on this website – go to contact us online and select medical request for an appointment – you will get a call or text once this has been clinically triaged. One form per person.

    • Only one medical problem per form per day
    • Only use one form per patient
    • Please use Pharmacy First if you can
  • Use your NHS app (going to ADVICE - Ask your GP for advice - press continue through the Accurx screen – the confirming that this isn’t an emergency – you are now at the same form)

Call reception

  • 020 8669 7612 – reception staff can fill in the same form for you – but we hope that if you can use the online form yourself you will. Keeping the phone lines for those who do not have access another way.
