If you need to access some information from your medical record, consider registering for enhanced online access, visit our Online Access page for details of this process. You can do this via our website and then you need to complete the consent form and drop it into reception with one piece of recent photo ID. This process may take a few days.
Alternatively, you must put that request in writing to your GP. Please detail exactly what you need to access (ie all or part of your records with dates) and this information will then be reviewed, discussed with a GP. You should allow 7 to 14 working days for this entire process to be completed.
