Fill in a form
- Fill in the form on this website – go to contact us online and select medical request for an appointment – you will get a call or text once this has been clinically triaged. One form per person.
Give as much information as possible, including availability.
The Clinician may give you an appointment with a GP, Advanced Nurse Practioner, Physician Associate or a Practice Pharmacists, GP Community Pharmacy Consultation Service.
- Use your NHS app (going to ADVICE - Ask your GP for advice - press continue through the Accurx screen – the confirming that this isn’t an emergency – you are now at the same form)
Call reception
- 020 8669 7612 – reception staff can fill in the same form for you – but we hope that if you can use the online form yourself you will. Keeping the phone lines for those who do not have access another way.
In Person
- Go to the Shotfield Medical Practice Reception. Where our reception staff can fill in the same form for you
Online appointments
Please note - forms are clinically triaged – appointments are given based on clinical need and capacity. Appointments may not be the same day.
For Nurses and other clinicians appointments
Just call the practice as usual for these appointments on 020 8669 7612.
Other Services
In addition to these services, the Practice also has a Health and Wellbeing Coach, Social Prescribing Link Worker and Physiotherapists, Mental Health Practioners - please phone for more information.

The GP CPCS (GP Community Pharmacy Consultation Service)
The GP CPCS (GP Community Pharmacy Consultation Service) is also now up and running. So you can call and for some ailments you can get an appointment with a pharmacist at a local chemist. This referral service is only available up to 12.00pm
